NOTE: To LOCK your booth reservation, all steps are required to be completed.

Step #3: Health Expo Exhibitor Agreement - CLICK HERE to fill out and submit this electronic form.
Step #4: Certificate of Liability Insurance - Obtain this form from your business insurance provider. CLICK HERE - For a SAMPLE of what this form should look like. Return completed form to sviehweg@sbgtv.com
