Thank you for joining us on Fresh Living! We are looking forward to your appearance on our show!
This guest form must be completely filled out and returned a minimum of two business days before your segment is scheduled to air. If this form is not received by the deadline, the segment will need to be rescheduled. This is to ensure all information is received and there is enough time to properly produce your segment.
Before filling out the guest form, please read this instruction page in its entirety as it has all the information you will need and will answer any questions you may have.
Arrival, Parking and Segment Instructions:
Please arrive at 9:45 am to allow enough time to set up and get mic'd up. We will tape from 10 am to 10:35 am
We suggest you park at the large parking garage directly EAST of the Wells Fargo building with entry from 300 South.
Our studio is located on 299 South Main Street in downtown Salt Lake City on the main floor of the Wells Fargo Building. Our lobby doors are red. On the pillar outside our lobby there is a phone. You can use that and dial ext. 1333 or call the news desk on your cell at 801–839–1333 and someone will come let you in.
Please be aware that in the event of "Breaking News" or "Developing stories", these times will change and there may also be occurrences where the entire segment will be rescheduled. This is rare, but please be understanding and accept our apologies for any inconvenience.
